Medical Release Papers
In order to ensure proper care in case of an emergency on the hike we are requesting that the wards provide 2 copies of the Medical Release Form for each girl. One for the girl to carry in her pack, and one for her leader that is going with her on the hike.
If there is anything written in the "Medical Information" section please write this on the wrist of the young women before the hike. (If you would like to ensure that it doesn't wear-off you could use  liquid bandage )
